My wife takes Nexium 12 hrs apart from Harvoni, according to recommendations you should be at least 4 hrs apart with anything that contains magnesium.
Snart
Helen 54 yr old female
Geno 1B since 84 from transfusion
started Harvoni on 11-22-14 with 8.5 million viral
After 8 days 933 viral and normal AST and ALT
Week 10 viral load  UNDETECTED
Week 17    detected 19
Week 18    detected 21
Week 20    UNDETECTED   Doctor did prescribe Ribavirin, but have not taken it yet as we are thinking that weeks 17,18 had to have been mishandled.
All other test have shown normal levels.
Week 21 results  on Tuesday 4-21
Week 4 post Harvoni 24 week plan UNDETECTED!!

Per the Harvoni prescribing information sheet Table 3 page 5

http://www.gilead.com/~/media/Files/pdfs/medicines/liver-disease/harvoni/harvoni_pi.pdf

Ledipasvir solubility decreases as pH increases. Drugs that increase gastric pH are expected to decrease concentration of ledipasvir.

Antacids (e.g., aluminum and magnesium hydroxide) Tums, Rolaids

It is recommended to separate antacid and HARVONI administration by 4 hours.

H2-receptor antagonistsc (e.g., famotidine) Pepcid, Tagamet

H2-receptor antagonists may be administered simultaneously with or 12 hours apart from HARVONI at a dose that does not exceed doses comparable to famotidine 40 mg twice daily.

Proton-pump inhibitorsc (e.g., omeprazole) i.e. PRILOSEC and NEXIUM

Proton-pump inhibitor doses comparable to omeprazole 20 mg or lower can be administered simultaneously with HARVONI under fasted conditions.

So basically Tums and similar no closer that 4 hours befor or after.

Pepcid and similar simultaneously and 12 hours after only

Prilosec and NEXIUM and similar ONLY AT THE SAME TIME as Harvoni on an empty stomach

Tums and similar must have a 4 hour window either side of taking Harvoni the problem is the antacid characteristic of the medication Ledipasvir requires an acid environment to be absorbed. If you lower the acid you lower the amount of Ledipasvir absorbed into the blood stream.

The prescribing info says "Antacids (e.g., aluminum and magnesium hydroxide)" the aluminum and magnesium hydroxide are listed as examples but the category of medicine is antacid irregardless of the specific formulary.

See prescribing info sheet table 3 page 5
