Good and Interesting Test Results

Six week labs done this morning.  One of the good things is that they post patient results on Mayo patient portal.  So I don't have to wait to talk to somebody to get results, they just post them when ready.  I had most of the results after about 4 hours, but it took about 15 hours to complete and post VL results.  Here they are:

AST=24
ALT=22
Bili=1.6 (crap, Gilbert's disease)
HCV RNA Detect Quant S=Undetected

Did six week labs because four week labs came back detected, less than 15 IU/ml. So I figure that sometime about a week and a half ago the last little virus went screaming into the night, hopefully forever.  I am, of course, thrilled. Still feeling no side effects and as of yesterday, I am half way done. 

I also noticed that as part of the four week tests they ran a test for IL28B polymorphism.  It came back CT.  Not sure why they wanted this parameter.   They have to send my blood sample to main clinic in Rochester, MN because they can't run it here in Arizona so it must be a PITA. From what I can tell, it's an indicator of treatment effectiveness when using interferon/ribavirin combos.  Evidently, CT and TT genotypes have poorer viral response compared to CC in terms of interferon/riba therapy.  All I can say is I'm glad I'm doing Harvoni.  If anybody is interested in the full text of the explanation of this that came along with my lab results, let me know and I will post it in this thread.  After I saw this IL28B thing, I searched these threads and learned a little more, but it's still a little unclear why my hepatologist ordered that determination.  I plan on asking. 

I need to get to sleep, but I'm too wired from these good results.  As always, more healing vibes sent to all.

Thanks Islandgirl.  You asked for it.  Below is the explanation that went along with that test result.  Again, not sure it's telling me anything since we're not talking about interferon/riba therapy.  But maybe it's implying that CT people like me may have a harder time with relapsing under Harvoni.  We'll see.

"IL28B-related rs12979860 CT genotype has been associated with a poorer viral response in HCV genotype 1-infected individuals treated with combined pegylated-interferon and ribavirin therapy as compared to CC genotypes. Individuals with the CC genotype, which is not the situation for this patient, have approximately a 2- to 3-fold greater rate of sustained virologic response (SVR) in HCV genotype 1-infected individuals treated with combined pegylated interferon and ribavirin therapy as compared to either the CT or TT genotypes. Similar SVR have been observed across various racial groups including European Americans, African Americans, and Hispanics. CC genotype has also been associated with a 3-fold increase in rate of spontaneous clearance of HCV. IL28B genotype is only one of many factors that can influence response related to pegylated interferon/ribavirin combination therapy in HCV genotype 1 infection and should be interpreted in the context of other clinical factors. -------------------ADDITIONAL INFORMATION------------------- Direct polymorphism analysis of the IL28B-related rs12979860 polymorphism is performed by a Polymerase Chain Reaction (PCR) based 5'-nuclease assay using fluorescently labeled detection probes. This DNA testing will not detect all the known mutations and polymorphisms of IL28B. Absence of mention of a specific gene mutation or polymorphism does not rule out the possibility that a patient has that or another variation that can impact the function of this gene, drug response or drug side effects. CAUTIONS: If the patient has had an allogeneic blood or marrow transplant or a recent (i.e. less than 6 weeks from time of sample collection) heterologous blood transfusion these results may be inaccurate due to the presence of donor DNA. For liver transplant patients, the IL28B genotype of the recipient and the donor are independent predictors of sustained virologic response with combined pegylated-interferon and ribavirin therapy. Thanks, Bob.  You gave to give credit where credit is due - Mayo's is certainly thorough.  While it doesn't apply to me either (1b), someone else may find it of interest too. 

I get new labs next week and see doc the following - hoping I'm still UND after 12 wks of Harvoni - now one month after EOT.  Holding positive thoughts for us all!  ....Islandgirl 8)

Congrats on that good result IG!  I'm sure you have banished HCV forever. 

I had a nurse call me today to give me the results I already knew.  She was very upbeat.  I asked if she found her job easier these days with all the breakthrough treatments.  Her response, "absolutely!" 

I'm so happy to read about all the positive results in these threads.  Even those who are struggling a bit seem buoyed knowing that there are better days ahead.

My doc and nurse are feeling like yours!  They are, however, frustrated with how difficult it is to get people approved and are trying hard to get more of their patients through process.  Good caring people and I'm fortunate to have them!
