First Post- 4 Week Results

Hello,

Hoping for some direction or guidance.  My Dad has stage 4 cirrhosis and is on Harvoni for 12 weeks. We received his four week results yesterday and he went from 6 logs to 2.1 logs.  I was told that they were hoping for undetected after for weeks but are pleased with the significant drop.   Has anyone else had this type of result and are  now undetected?  The significant drop is still a good thing?

This is way to early to worry. If he is showing any improvement its good. Results are slightly different for some. Do a little search and you will see a lot of members are undetected at 4-6 weeks and some are now still showing very low almost undetected VL at EOT but at SVR 12 ( 12 weeks post tx) are undetected.

Overall the success rate seems to be very good. have your dad stick to it , finish the course and he will probably do well.

Yes, the main thing is to focus on the end game and do what the dr tells him,
I'm in similar as your dad, I'm at 9 weeks and so far I'm undetected and doing better,
Is your dad still working ? Be encouraged, the meds are working, time will tell...

Stay positive and learn all you can to help your dad.

Hello,

He's been in and out of hospital since August of last year. He's had GI bleeds as well as taps on his abdomen.  He's also paraplegic.  They are now giving him strong antibiotics for a urine infection and bone infection from pressure sores.  Two of his main doctors are positive but then we meet residents in hospital who don't know the full picture who don't seem interested in the plan we have for him.  I'm taking the result as a positive Because it's a significant drop, just worry about the meds they're giving him and his other health issues.

Stay positive and proactive. If your dad is ill and on other meds be persistant with his care team and be sure they have taken into account he is on Harvoni. Harvoni is a very new drug and not every combination of interaction with other meds has been explored so you really need to have a good GI team that is active in his care to be sure that everything is working towards a positive outcome.

Im on a lot of maintenance meds and wen following up I have not always been able to get absolute answers either from my MD or Gilaid Support because they will say it is not been tested so we have to use some amount of common sense and in my case I minimize any med or d/c it until Im done with the harvoni if I cant get a direct answer that they have researched it with harvoni.

KJG:  I believe I read somewhere that if after 4 weeks you have a 2 log drop, they consider that a strong responder, so your dad is doing very well.  At 4 weeks my test was VL 59.  I am one of those that tested <15 IU/mL but detected at EOT and at 8 weeks post treatment came back Undetected!  So your dad is doing good.

I desperately wanted to be und at 4 weeks but I wasnt. I dropped from 340,000 to 40 by week 4. I was pretty bummed, but all other markers (ALT, AST, GGT etc) continue to fall so I take comfort in that and in knowing others have been in my shoes.

Stage four cirrhosis is high for only getting 12 weeks, 24 weeks maybe needed which is recommended for this scenario. The problem is that Harvoni doesn't just go into a working mode because you swallowed a pill. Harvoni works after the liver metabolizes it. If you have cirrhosis and your liver is not working properly it cannot metabolize the Harvoni as good so you need to take it longer.

Speaking for all the nerds in here, "6 logs" means a viral load of about 1,000,000.  "2.1 logs" means a viral load of about 126.  That's clearly a significant drop in VL and on par with what many of us have seen. Congrats to your father.

 A logarithim is a short cut for really big numbers.  If the question put to you is "How many numbers do we multiply to get 7.924.270 with a base of 10 (how many 10's)?   You basically count how many 10 places and in the case of millions it is 6, (10 million or more would be 7)  It is the reverse of squaring or cubing....(exponential) a number.

Example How many 2's (base of 2 instead of 10) do we need to get to 8?

2 X 2 X 2 = 8 so the answer is log(2) = 3

Base 10 is usually used in science or medical calculations and it just makes dealing with very large or very small numbers easier to deal with and that is what they deal with when looking at how a person is responding  to treatment.

 So using Log(10)
10 X 10 = 100 (or 10 squared)   Log(10) = 2 
10 X 10 X 10 (or 10 cubed)  Log(10) = 3

Today he has encephalopathy. Not a good day. He has decompensated cirrhosis and 30% function left. i hope that Harvoni will work and he can get somewhat better and be less symptomatic but days like today are difficult

"For a log calculation fill in the boxes like this:
log [10]  [number]
Hit calculate

For example, if viral load is 345678 and you want to know the log
Put [10] in the small box and 345678 in the number box, hit calculate and we get log 5.54 (rounded up)

For an anti-log calculation:
Put log number in the small box and hit calculate.

For example, you put 5.54 in the very small box, hit calculate and we get 346737

Slight difference is because we only used two decimal places."

I've been told his prognosis is poor even with Harvoni. Still not given a life expectancy.  I've been told that the pressure ulcers are the problem and wouldn't be considered for a transplant. They are planning on operating on the sores after Harvoni but who knows if he would survive the surgery

Good Morning Genotyoe 3 friends. I have had a great time reading all of your posts!
I am in my 6th week of treatment. At week 4 my viral load has come down from 8 million when I stared treatment to 15 (just 15) at week 4.
I think that is awesome.
I have been treated twice before with
Interferon alone 1 shot a day
Then with the rib and interferon 1 shot a week.
I do have a few issues with the treatment, nothing like with the interferon though.
Dry eyes
Itchy at night, that seems odd to me
I feel very tired, that is getting to be a little worst now. Have you all noticed that?
Also, I am very pale, odd because I am in the sun by my pool everyday for an hour or so, that is all of the sun I can handle since starting the meds.
If anyone has any insight please let me know.
