My Support Path Program from Gilead

Now that my insurance company decided to pay for Sovaldi & Ribavirin I still want to go in the My Support Path Program.  But I look at filling in that form and it appears that it is more for helping with appeals and their program to pay for the medication.

Do I need to still fill that form out in order to get the other benefits of the program?

Hi RWE,

I signed up for the coupon(s) and that handled the rest of the program since I didn't need any other assistance. I have had a couple of calls from them to check and see how I am doing in the past 3 months.

SVR 12 is 7/23/15 ACHIEVED!!! 
and SVR 24 ACHIEVED!!
