Virologic Breakthrough on Harvoni week 17
« on: March 29, 2015, 03:51:14 pm »
I cant believe what we are looking at today, Helen's viral load had been undetectable for the last 7 weeks and then all of a sudden on week 17 she gets a 19, what in the world does this mean? I cant talk to doctor until tomorrow and this is causing terrible fears....
I have researched this and it appears almost impossible, all Gileads trials show maybe 1 experiencing this.
Any thoughts?
Thanks
Snart
Helen 54 yr old female
Geno 1B since 84 from transfusion
started Harvoni on 11-22-14 with 8.5 million viral
After 8 days 933 viral and normal AST and ALT
Week 10 viral load  UNDETECTED
Week 17    detected 19
Week 18    detected 21
Week 20    UNDETECTED   Doctor did prescribe Ribavirin, but have not taken it yet as we are thinking that weeks 17,18 had to have been mishandled.
All other test have shown normal levels.
Week 21 results  on Tuesday 4-21
Week 4 post Harvoni 24 week plan UNDETECTED!!

I'm so sorry to hear that!

A couple of people or so have experienced similar, I believe.  Go to the News section and I think you may find discussions that could help.  I tried to copy a link:  http://forums.hepmag.com/index.php?topic=2383.msg17078#msg17078  If that doesn't work just try the topic just under "Post Treatment" category.  Good luck and you guys are in my prayers.  ...Islandgiril

Just wanted to give this thread a bump to gather more comments, and to state that many of us are wishing you well with your situation and are most anxious to hear what the doctor has to say.  I do know that with Harvoni, on-treatment testing has limited value relative to final outcomes as determined 12 wks post treatment and beyond.  So Id try and not freak out too much over this.  Still, like you, Im puzzled as to how viral loads can go UP during treatment with Harvoni.  Interestingly, depending on the test, if this were the Gilead trial, since her result is still < 25, it would not have been logged as a Virologic  breakthrough.

Ma Snart,
I had a similar issue. I was undetected  from week 3 then on week 8 I had a detected! Lucinda told me not to freak out as this may be an abberant lab result or a lab error. I repeated it 4 days later and it was undetected and has been ever since. Hope this helps! Larry

I am on my last week on Harvoni and am ND but my Dr told me up until my 6 more follow up I am not considered cure.  A small part of virus may live in a muscle or small part in your body.  But I would breathe and have another test.  God bless

One reason why UND At week 7 could become a 19 at week 17 could be that the real level of UND might have been 14 or less and normal fluctuations brought it back up to a detectable 19 later on (assuming a test with LLQ of 15...
I wouldn't worry too much with a 24 weeks treatment because nobody (or close to) relapsed. 24 weeks is the "Cadillac" treatment so, give it a chance!
60 years old. Likely infected 1975. Geno 1a
F4  8 millions VL,  AST 140  ALT 140
Generally in good health except problems are creeping up rapidly!
2 failed attempts Inf 2000 and Inf-riba 2010
Harvoni 24 weeks
Start 11/13/2014   EOT 4/27/2015
VL2 - UND
VL4 - UND (may 25)
VL12 - UND
VK24 -  UND
Hopefully cured forever!
