2 yrs SVR now This

Been quite a while since I have posted 2 years SVR with Epclusa after Harvoni failed.
Fiboscan 6 months ago 9.4 kPa down from  12.5 kPa last year. My recent labs show a result of ALT @22 U/L - AST @30 U/l  a year ago it was ALT 21 AST 25 . And now from my Ulrasound reports:

There is a 0.8 cm echogenic lesion in the right hepatic lobe which is not identified on previous examination.

 Feeling sick about this. especially after my liver has shown to  improve.

ALT and AST vary all the time as long as they are in normal range not an issue.

There are many non cancerous liver lesions so hang in there I assume you will be having additional imaging like an MRI? Best of luck that it is not a concern.

Thanks for the positive reply Lynn. Checking further I see   only 3 types of Non-Cancerous Tumors ,Hemangioma, Liver Adenoma and Granuloma.   Hemangioma and Adenoma most commonly occur in females. As I'm a male with cirrhosis that leaves me with Granuloma.

Well most commonly perhaps but not exclusively

And non cancerous is more common than cancerous

My general feeling in life is hope for the best especially when it is the more likely option

Thanks Lynn,

So the chances are greater that it is non cancerous  more so then cancerous even with
Cirrhosis? My fibroscan was a 9.4 rating according to my Hepatologist,  that would be a F3  down from F4 previously.  He was extremely pleased. 

As of now my heads kind of spinning...I'm trying to keep calm and not upset my wife. I exercise daily, eat right so I'm a little confused nervous and angry. My Hep Doc is 500 miles away and no one is answering through  portal.

I went ahead and scheduled my MRI for Thursday morning the soonest I could get in
My GP is a  Internists and had already sent in the order to the Imaging Center.

Do you have the AFP blood test done? I get that every 6 months along with an ultrasound. AFP I’d elevated may indicate liver cancer but can be elevated for other reasons so it’s not a perfect indication and should be used in concert with ultrasound.

But anyway if AFP is elevated like several hundred or higher that would be something you would want to consider.

For reference mine was in the mid 20’s when I had hep c but now is 8.9 with normal at 8.3

Yes statistically they are more likely not cancer but the possibility does exist so it does need to be investigated further to make sure hang in there I’m hoping for good news for you.

No they failed to do the AFP... dropped the ball here locally. So the Liver Clinic  called to have my Internist see that lab up asap. Also finally reaching someone from my Liver Clinic   my hep Doctor says he was not too concerned at this point as I'm no longer cirrhotic and it happens often to patients  thus the purpose of the six month Ultrasound. And as I already setup the MRI it  will give him more info.

The AFP will give you an idea especially if it comes back similar to previous ones and not very elevated

Update:

Just received MRI results:

CONCLUSION:     
No suspicious hepatic mass identified.  No correlate is identified for
the area of hyper echogenicity seen on recent ultrasound.
Nodular contour of the hepatic margins, suggestive of cirrhotic change.
Small 4-6 mm enhancing foci within the gallbladder lumen, most
compatible with polyps.
Duplication of the partially imaged left renal collecting system. 





If I'm reading this right it is good news. I have not recieved the tumor marker lab yet
but I guess the point is moot at this point.

Lynn those  gall bladder polyps have been there for a while and havent changed he was not  too concerned about them. I'll share my fibroscan from last Sept as I  often see questions regarding folks with cirrhosis questioning about liver improvement on this forum. Keeping in mind I eat right and exercise.  I was 12.5 kpa which is F4 of course :

Your Fibroscan performed today showed a value of 9.4 kPa with
20 % IQR, which is suggestive of stage F2 - F3 liver fibrosis/
scarring. CAP Score 279. This is MUCH improved!

Nice! What is a CAP score?

My pretreatment Fibroscan was 27 one year post 44 and 4 years post 29 so not much happening as far as improvement for me :(

Ok I just looked up CAP score I don’t know mine but I had fatty infiltrates on my last ultrasound and my doctor did say my Fibroscan indicates beginnings of fatty liver.

I joined a gym today now I just need to show up and exercise lol

Can I just ask (Lynn) -

Should everyone who's been through Tx get a follow-up ultrasound?
I've only had one done - two or three years before Tx.
From memory it was about 8 kpa (F2 - F3) then.
They don't seem to be prescribed as a matter of course here in NZL.

P.S. You can create your own exercise regime if you can't face the gym!

Well at least per the AADLD post treatment recommendations they say for those who were F3 or especially F4 before treatment they recommend 6 months abdominal ultrasound and AFP blood testing as even though our risk is reduced we remain at increased risk for HCC as compared to those with less liver damage
