which insurance companies use CVS/Caremark as their specialty pharmacy, which ones don't?  where can i find this info please? Can anyone help me with any info you have?

Only insurance that use CVS/Caremark as their specialty pharmacy will cover Harvoni as of January right?

I really can't answer that.  You have to ask your doctor which is his speciality pharmacy and he should take care of it.  In my case, my doctor  sent a letter to his speciality care pharmacy and to my amazement, no work on my part, they called me a day later and said I was approved and also I would not have to pay a co-pay and they take care of the shipping.

Ask your doctor, he should know and he should do the work; if not, go to another doctor.

You might want to call CVS and ask them for a list of plans/insurers they work with. You should be able to get the information you need.
