I talked to my doc today about the same thing. The riba is playing with my hemoglobin and rbc.  We will take labs very 2 weeks and watch the results. We came up with a plan if need be reapply for another 12 weeks of Harvoni and then try to sneak in the riba. The key is to stay connected to the doc and play the insurance like they play us.
I have been lucky in that every time I have applied for any meds they have approved them. My company did change to Aetna this January so I got lucky because we had UHC?  I have always tried to be ahead of the curve and have a plan in case.
Good luck
