Progression to F4 or Cirhossis !

I have had hep c for the last 10 years or so.
Just did the fibroscan and the result was F2/F3, borderline F3, 14.8 kpa.
I dont drink alcohol for the past 13 years. I am genotype 1a.

I plan to start Harvoni as soon as its released in India as I will be there for a while starting Jan 2016.

How quick or slow is the progression to F4 from F3 ? I know its guess work here but just asking to see what kind of  time I have on my hands.

This maybe an odd question but I needed to know and ask.

In my country it costs € 60000 so the state wont pay as the fools still feel I am healthy thus India is my best option for the very near future.

According to this scale you would be considered F3/F4 rather than F2/F3. I don't know where you come from but with a score of 14,8 kPa it seems obvious that you need treatment. Maybe you should contact your doctor again?

yes correct thanks. I live in North Europe and as written above plan to travel to India ASAP but harvoni generic has not been released there yet but I hope it is this year.

Hello Jakas,

Fibrotic progression is very slow and can even stay the same or slightly regress spontaneously.  I am 14.5 which is borderline.  Once you clear the Hep C, you will remove the full burden.  There are studies and stories of reversal if the Cirrhosis or fibrosis is not to far along.  There is another thread where about 5 studies are posted that are encouraging.  Just cure the C first.  Then there is hope for the other.  One older study follows cirrhotics from Alcohol, Hep B and Hep C that quit drinking or, were cured of hepatitis and there was a fair percentages of them that experienced partial reversal. I had 40 years of C bearing down on me and drank through some of that time though moderately.  The 14.5 score was from a fibroscan.  17 years ago I was at F2.  So slow but I know folks that are non cirrhotic and have had HVC for 45 plus years.  Everyone is a little different.  Good luck, you are going to be OK.  :)

I have been followed since I was diagnosed with hep c in 1990. I had liver biopsies every 5 years since 1993. With each successive biopsy I went from F1 to F2 to F3 and finally in 2008 F4. Basically I increased one fibrosis score every 5 years. But everyone is different some have hep c for 20 years or more and are still F1 so really there is no way to predict if you will ever progress to cirrhosis.

The only thing you can do is be kind to your liver and avoid causing harm to include being at a normal weight as fat cells in the liver can also cause liver injury.

Yeah I know easy to say I could lose 20 or more pounds myself but we do the best we can.

From what I have seen F3 is treated the same relative to being able to get treatment as F4 so being F3 should prioritize you to be treated.

Jakas, I'm also in a situation that treatment in my country is far from being even approved, and even then probably they will approve tx only for cirrhotic ppl.
Everyone expect Indian generics for Harvoni, but that might be delayed due to the big pharma pressing Indian regulators to insist on trials first. That alone can postpone sales for at least half a year.
There is another option but so far I'm not sure how to get it other then travel to Bangladesh. Bangladesh company Incepta Pharmaceuticals started selling Twinvir which is their name for Harvoni.
Earlier this year they were also first to deliver generic Sovaldi beating Indian companies by few months.

They did change their policy a few months back for those with insurance who have been denied by their insurance but yes they do still provide some help with appeals and I think maybe for those without insurance

since I posted to your opening comments I had a fibroscan done and it showed a reversal from F4 to F0-1.  I have seen others have similar drops after the burden of Hep C was gone.  Good luck on tx and keep us posted.  Everybody is with you and congrats on the meds.

     Congratulations. I don't know if your results are common, but that is great. That would be my greatest wish.
     And to jakas, I feel such a change in your confidence. I agree, this is a great place to be connected with. And the degree of genuine compassion, not idiot compassion because you feel uncomfortable to talk about HepC is amazing. No comparison anywhere. As I have expressed before.. we have done or are doing battle with this virus. We are warriors in our right. Proud.
     I have experienced a spookiness, you might say to this forum. Today is my last day of Harvoni. I am nondeductible right now. But that doesn't hold the confidence it once did. I was here before with Sovoldi. I was doing the happy dance. I envisioned a life without the virus. Then my six month test showed a relapse. Cirrhotic tissue can prevent the medicine from getting in contact with all the viruses  So I was starting to get pushed down. Not being a warrior. Bam! this thread appears in my email. I read beto and jakas last post and a little sense of the rainbow starts to appear.

If it helps I treated 3 times with interferon based treatment and was a null responder. I treated from March to June with olysio/sovaldi for 12 weeks (probably should have been 24 but that was changed months after I completed). 12 weeks later in Sept 2014 I was found to have relapsed even though I was not detected from week 4 to EOT. Needless to say I was crushed.

I started Harvoni for 24 weeks 18 Nov 2014 in January 2015 we added ribavirin for 15 of the 24 weeks of Harvoni. In July at 12 weeks post I was still not detected and the same at 24 weeks post in October.

So just to let you know you can beat this thing and Harvoni is awesome
