SVR2? ALT/AST Ratio?
« on: November 10, 2015, 05:18:03 am »
Did anyone get SVR2? Is it 'indicative' of anything? I know there is this number of SVR4 being 99.8% indicative of SVR12... but what about SVR2?

Also, does ALT/AST ratio matter if the ranges are within normal, or does that 2:1 bad ratio only apply if the values are over normal?

I have a 12 ALT and 24 AST (thus 2:1 ratio) but it's been like this since I first became UND, 2 weeks into the TX.
GT1a - TX-naive

Baseline:

16.2 kPa
ALT: 61 iu/L
ALP: 74 iu/L
Albumin: 38g/L
Bilirubin: 27 umol/L
Platelets: 80
VL: 792,000
History of 4 violent varices bleeding episodes and splenomegaly present.

21/07/15 - TX start - Harvoni + Ribavirin 12 weeks.
Week 2 - UND
Week 4 - UND
Week 8 - UND
13/10/15 Week 12 - UND
19/11/15 - SVR4
8/1/16 - SVR12 !!!!!

I had an SVR2 done as well but I don't think it's used as any type of predictive tool. At two weeks EOT, I think most people still have the harvoni in their system meaning that most everyone would still show undetected at that point.

SVR4. SVR12 & SVR24 are the main ones used to determine a sustained response and it's my understanding that SVR24 is on it's way out since 99.8% of the SVR12's go on to be undetected at 24 weeks. From Gileads Harvoni site

Cure means the Hep C virus is not detected in the blood when measured three months after treatment is completed.

Though, an undetected SVR2 is nice to look at, due to the fact it means your halfway to SVR4, I don't think it's used.

As for the ALT & AST the AST can become higher than the ALT just by working out or straining your muscles. My 4 week EOT showed my ALT at 23 while my AST was 35 but at the time I was going to physical therapy. At the 12 week mark my ALT & AST were both at 19 - 19.

In two more weeks, or at your four week post EOT, you will have an answer you can use. 12 ALT and a 24 AST looks good at this point.
Pre Tx (VL 12043488) ALT 52/ AST 39
Harvoni Start Date 03/12
2 Week in Tx Results HCV RNA Detected, less than 15 IU/mL. ALT 19 / AST 19
4 Weeks in Tx Results HCV RNA Not detected. ALT 21 / AST 18
12 Week in Tx Results HCV RNA Not detected. ALT 17 / AST 19

Post 12 Week EOT Tx ALT 19/ AST 19 (Undetected)
Post 24 Week EOT Tx ALT 19/ AST 15 (Undetected)
