Harvoni and HCC connection?

Hi Everyone,
I have noticed and read of a few individuals in this form that discovered they have HCC towards the end or after treatment.
These were mostly all stage 4 cirrhosis patients.
Most likely they would have developed, or were already carrying the HCC mutations before the start of the Harvoni treatment.
But since the drug is still so new, and I happen to be stage 4, I am curious if there is a connection or an increased incidence of stage 4 cirrhotic patients using Harvoni and HCC?

Specifically, AFP serum markers in the 400-500 range are an indication of "probable" HCC. Although Ultra Sound is more conclusive evidence.
So one way to monitor a connection is for those taking Harvoni to pay attention to their AFP levels before, during and after treatment. Mine were in the 200 range pre treatment, but Ultra Sound showed no lesions. Doc said AFP in that range is inconclusive of HCC. Again stating 400 to 500 is typical for HCC.
IS anyone else monitoring their AFP levels? Thoughts or articles?

Started week 4 Harvoni and so far sides are mild.. I have noticed working out, at least walking for 45 min a day REALLY helps. Along with the mandatory 2 quarts or more "water irrigation."

I'm not an expert on HCC so take all this with a grain of salt. It's just my thoughts written down, worth exactly what you paid for them.

My AFP was 76 at the start of Tx. It had been climbing over the years. At 12 weeks post treatment it was 8. When I asked my specialist what a level like my starting one meant he said- to him, an AFP like that was indicative of cirrhosis, that's about it. A drop is great of course, but my beginning Tx number didn't seem to worry him overly. Normal lab values for my lab are 0 - 7.

My u/s 12 weeks post showed no changes from the previous one in April, nothing untoward except of course all the crap that goes with being cirrhotic.

I walked every day and like you, found it an excellent therapy!

Greg, in the USA the protocol for people with F4/cirrhosis seems to be an AFP test and an ultrasound every six months. My hepatologist follows that protocol and says HCC is very treatable if caught early. AFAIK that protocol is the same no matter what treatment the person is/was on.

Different labs have different normal ranges for the AFP serum tumor marker. At my PCP's lab, normal is <6.1 NG/ML. My personal results were 5.3 in March (several months before I began Harvoni Tx) and 4.6 in mid-September (when I was nearing the end of Tx). Thus, in my case, Harvoni Tx made HCC less likely.

Glad your sides are mild. I, too, fared better by getting exercise and staying well hydrated. Have a wonderful holiday season.

Yeah it is a cirrhosis HCC connection not a Harvoni HCC connection in my book. From what I have read if you don't yet have cirrhosis you are not at risk for HCC. Cirrhosis is what puts us at risk for HCC.

I have been getting my AFP and abdominal ultra sound every 6 months since Jan 2008 as everyone who has cirrhosis should per the protocols for monitoring patients with cirrhosis.

My results before I treated were 26.6 & 26.3

After I finished Sovaldi/Olysio in 2014 and before I found out I had relapsed it was down to 14.9

When I started Harvoni it was at 15.6 and 13.4 when I finished.

My most recent 6 months post Harvoni and at SVR24 my AFP was 10.4 the lowest I have ever seen it with the standard range for my lab 0.0 to 8.3 ng/mL

So my liver is much happier than it has been in years and I am excited to see what the new year brings.

Hi Everybody and Happy Holidays!
I was actually thinking of starting a thread like this too.  Just to put it out there since I am one of those that developed HCC after starting Harvoni. 

I don't want to think that our miracle cure drug could possibly be linked to my HCC
What is odd to me is that I have had cirrhosis for a very long time and not until I started Harvoni did the HCC pop up.  I know it is probably a coincidence though.
And now with the lung lesions who knows.......

My AFP is 9 and I don't know what it was before or if they even checked it before.

I hope everybody with cirrhosis is getting their 6 month ultrasounds done in a timely manner.  I was 2 months late and have been waiting 3 months for a treatment plan.   >:(  ......so although diagnosed early enough ..................

Hi all,

From what my specialist tells me it is the cirrhosis that puts you at risk, more so when the cirrhosis continues to progress as with aggressive HCV or continued excessive alcohol. AIUI your liver will always attempt to repair itself and regenerate, but if it is sustaining further damage to the regeneration sites then that will drive HCCs. He put it to me that achieving SVR wouldn't totally remove the risk and I will always require monitoring but it would considerably reduce the risk which was one of the factors behind my decision to treat with generics with his script and support.
I was recognised as a risk previously so monitored 6 monthly via scans and AFP but when my LFTs started to rise again they put me on 3 monthly which picked the first up via AFP and scan at the next tests, then repeat 6 mths later. Latest 4 week Tx bloods show dropping AFPs.

We know there is a link between HCC and cirrhosis, and the only way to cure hep C and hope for a chance to reduce the HCC risk is with treatment. However, it doesn't always happen. As for a link between Harvoni and HCC, my theory is that people are discovering their HCC sooner if they are on Harvoni  or in post-Harvoni follow-up. I believe the HCC would have happened anywise, but it would have been caught later and that isn't good.

My specialist told me (and my GP) that by successfully completing treatment and having my AFP levels drop I had reduced my risk of HCC to around 1%. Now that may or may not mean anything to me - lies, damned lies and statistics etc, but I like to cling to it.

Deb...I am so sorry to hear - it must be so incredibly stressful for you now.  Good luck to you tomorrow.  Keep your spirits up as best as you can.  You can get through this...you dealt with the HCV all these years!

Hi everybody and HAPPY NEW YEAR!!
Thank you all so very much for your thoughts and prayers and support.
I found out yesterday that the lung biopsy came back clear and we are moving forward with the resection next Friday!! Finally a light at the end of the tunnel. :)
The surgeon said that the tumor is located on the outer edges and I won't loose but 10% of my liver which is way better than I expected.
My pain is still pretty extreme but hopefully it will be over soon and I can start fighting my way back to health.  (I haven't been able to eat much this past week or so so my body is pretty run down)

Again, your support is really really appreciated.  What woukd I do without you guys?

FABULOUS NEWS, DEB! I am so happy for you, and that you are finally able to move forward with a plan of action.

As for not being able to eat much and getting run down from that... I am sure you have tried smoothies - fruit, veggies, juice, yogurt... That is what I did during treatment when I had zero appetite. I still lost 15 lbs during & post-tx (which was significant for me since I was skinny to start). But the smoothies helped me nutritionally through that difficult time.
