Drastic Increase in Liver Enzymes after 4 weeks of Harvoni
« on: March 24, 2016, 09:46:36 am »
My ALT and AST were in normal range prior to beginning treatment.  After 4 weeks on Harvoni, they are now 888 and 1200.  Has anyone else experienced this?  Going back today to recheck this and will know this afternoon.  After a few days, felt better than I had in years, now very very tired.

Were you diagnosed with cirrhosis prior to treatment? Is your bilirubin also elevated? What has your doctor told you? Is your doctor a specialist (liver or GI)? Are you in the U.S.? Are you taking other medications? Could you have been exposed to hep A or B or suspect another liver injury?

Stage 2
Bilirubin .3 to .6 (In range) as well as everything else, but the ALT and AST.
Doctor is GI and has me going back for recheck today.  Said if numbers were the same he would want me to discontinue treatment and would then discuss next step(s).
I am in the US and only other med I'm taking is Lisinopril for blood pressure.  Don't think, I've been exposed, but, I suppose anything is possible.
Amazing how good I was feeling only a few days after beginning treatment, and how bad I feel now.
Thank you for your response....this is all so crazy!

Yes, it is crazy. However, drug toxicities occur for all drugs. I have not heard this happening except for those who already had decompensated cirrhosis. Your doctor can call the drug manufacturer or provider service, but sounds like the proper course of action.

Are you taking any herbs or supplements or drinking a lot of herbal teas?
Were you immunized against hep A and B?

Nothing, now.  Stopped everything but the BP when I began treatment.  Just drinking a lot of water and seem to still be dehydrated.  Hmmm, on the immunizations, don't think so.

Also, spoke to my GP and sounds like she may want to see me today to discuss all this.  I ask that she call in so I can also have cholesterol checked when I go today since I took myself off this med when I began treatment.

Let us know. If it helps, the liver is highly resilient and although these numbers sound terribly high, they can get substantially higher. And rest up - this can be taxing   -listen to your body.

Did you mention the immunizations? I am supposed to get a Hep A and Hep B vaccine.
Did you skip that?
Do I really need them?

Hi,

I'm interested in what Iporterrn asked about taking supplements while on Harvoni. Today is day 24 of my Harvoni treatment, and I also take about 1,000 mg per day of liquid Milk Thistle supplements. I informed my GI before treatment about taking Milk Thistle, and also listed Milk Thistle in my initial coverage request for Harvoni, which was turned down, and then I filed an Appeal, which also listed Milk Thistle. I was APPROVED for Harvoni through the Appeal. So, my big question is, Does Milk Thistle adversely affect the way that Harvoni works? Is it advisable to stop Milk Thistle while on Harvoni?

TopCat - we don't know. However, since milk thistle competes with some of the same drug metabolism pathways that antivirals use, I would not take the milk thistle.

Morab - definitely get those immunizations!
