Considering Hepatitis C Treatment / Closing the Hepatitis C Treatment Gap
« Last post by iana5252 on December 18, 2024, 10:46:06 am »
Rachel Melson, DNP, FNP-C, has a tale to tell. The nurse practitioner is the director of the outreach clinic at Swope Health in Kansas City, Missouri. Her tale provides insight into why, according to the Centers for Disease Control and Prevention, in 2022 only one in three people diagnosed with hepatitis C virus (HCV) received treatment within 12 months of their diagnosis. And this, even though for the past decade, a new era of hep C treatment not only cures the vast majority of people but is also almost completely free of side effects.

Swope Health treats many people who are currently or formerly homeless or have experienced imprisonment and drug use. Recently, says Melson, a nurse on her staff spent 35 minutes on the phone with an insurance company trying to get prior authorization to cover someone’s HCV treatment. Insurers often set high barriers for the treatment, which costs tens of thousands of dollars per course.

Two experimental thyroid hormone receptor agonists, VK2089 and ALG-055009, led to significant improvements in people with metabolic dysfunction-associated steatohepatitis (MASH), according to late-breaking study results presented at the American Association for the Study of Liver Diseases Liver Meeting 2024.

One study showed that up to 75% of people treated with VK2809, from Viking Therapeutics, achieved MASH resolution with no worsening of liver fibrosis at 52 weeks. In another study, Aligos Therapeutics’ ALG-055009 led to a reduction in liver fat at 12 weeks, supporting further evaluation.

On March 14, the Food and Drug Administration (FDA) granted accelerated approval of Madrigal Pharmaceuticals’ resmetirom (brand name Rezdiffra) as the first medication for metabolic dysfunction-associated steatohepatitis (MASH), or advanced fatty liver disease.

Resmetirom, a once-daily pill, was approved along with diet and exercise for adults with MASH who have moderate to advanced (Stage F2 or F3) liver fibrosis but have not yet progressed to cirrhosis (Stage F4). Patients will not need a liver biopsy to be eligible for the new treatment. The approval is supported by results from a late-stage clinical trial which showed that resmetirom is the first drug to both reduce liver fat accumulation and improve fibrosis.

Hi Jack!

There was a delay in me getting the lab order, so I just got my final viral load test results in this morning.

Not Detected!!!!!!!!!!! I AM CURED!!!!!!

Tears of joy. I wasn't sure I would ever get here. The dragon is slayed!!!

Thank you so much for all of your encouragement, good wishes, and fingers and toes crossing. I so appreciate you sharing your experiences and wisdom that only comes from one who has been on this journey before and at last reached this destination.

I hope my journey gives others hope that even for previous non-responders, a cure is possible.

Best of everything,
Annie
